It always makes my thumbs cramp up when I am giving a massage.
Though, it is wore for me when the person is laying down, since I would have to bend over and all of that. I find it easiest when you are sitting beside the person, or behind them so you are at the same level.

I find it harder when the person is sitting up because their back muscles are strained, or tight.
When they're laying down, it's loose and easier to get at the knots.
