Rainbow skull comes from Ancient Bundle of Death. You also get a nice galaxy robe to go with it.

I don't see the rainbow flame eyes.

The skull itself has black eyes... missing opportunity for flaming rainbow eyes!

The rainbow eyes are definitely from Eye Sore, sorry, not eyes of nature.
