Mind posting a screen shot of the item on your layers with the avatar preview as well, and one after you've saved your avatar?

Sometimes setting up my avatar messes up for me too.

I've noticed that it takes a minute for items to equip, sometimes. So if you save to quickly it won't add the new items.
